Referring party submits the referral packet with all required documentation, including behavioral, medical, educational, and legal history.
Our team reviews the packet to assess fit, safety, and clinical appropriateness. We may request additional information from providers.
Introduction meeting between the youth, family, referring party, and our team. Site visit is offered when appropriate.
Acceptance decision is communicated, admission paperwork is completed, and transportation and start date are coordinated.
The youth arrives, meets the team, tours the home, and begins orientation. Individualized service plan is finalized within the first weeks.
A complete packet helps us respond quickly and place well. Missing documents can delay review, but do not stop us from starting the conversation. Reach out any time, even if the packet is not yet complete.
